• A public utility water pipe or gas/oil pipe being used as the means of earthing
• Cross-sectional area of the earthing conductor not meeting adiabatic requirements
• Inadequate main protective bonding conductor (<6 mm²) or evidence of thermal damage

Supplementary Bonding Issues

• Absence of supplementary bonding where required (e.g., locations containing a bath or shower), unless the following:
  Three conditions are satisfied:
  1. All final circuits comply with Regulation 411.3.2 for automatic disconnection
  2. All final circuits have additional protection by a 30 mA RCD
  3. All extraneous-conductive-parts are effectively connected to the main protective bonding
